Which emergency department for a child

Where the paediatric emergency care actually is in this region, the age rules that override everything, and the signs that make a child’s illness a same-hour problem.

Children are not routed like adults

Most general hospitals in the region will assess a child, and all of them will resuscitate one. What differs is depth: paediatric emergency medicine, paediatric surgery, a children’s intensive care unit and the specialists behind them exist in a handful of places, and the ambulance service knows where. In Estonia the dedicated children’s hospital is in Tallinn and the children’s clinic of the university hospital is in Tartu; Helsinki has a purpose-built children’s hospital in the HUS group; the Swedish, Norwegian and Danish university hospitals each carry the paediatric load for their region. For anything serious, the destination decision belongs to the emergency service rather than to a map.

The age rules that override judgement

Below three months, any measured fever of 38 °C or more is an urgent medical matter and not a home-care one, because a very young infant can be seriously infected while looking almost well. Between three and six months, a fever above 38.5 °C, or any fever with poor feeding, drowsiness or a rash, is a call. Above that, the rule stops being about the number and starts being about the child: an alert, drinking child at 39.5 °C is less worrying than a listless, poorly drinking child at 38.2 °C. TempoLife has a whole reference on this, including the weight-based dosing ladders, at fever in children; use it for the dosing and this page for the destination.

The signs that mean now

Call the emergency number for: a rash that does not fade when a glass is pressed against it; breathing that is fast, grunting, or drawing in below the ribs or at the neck; a child who cannot be roused or who is unresponsive; a seizure lasting more than five minutes or a first seizure of any length; blue or grey lips; a stiff neck with a fever and light hurting the eyes; a baby who is floppy or will not feed at all; and any suspected swallowed battery, magnet or medicine. Dehydration is the quieter one: no wet nappy for eight to twelve hours, no tears, a sunken fontanelle in a baby, or a child too tired to drink. Vomiting that is green, or abdominal pain with a swollen, tender belly, needs the same-day assessment.

What to say, and what to bring

The four things a clinician wants immediately are the child’s age, the child’s weight, what medicines have been given and at what times, and what the symptoms have done over the last few hours — better or worse, and how fast. Bring the child health record if the country issues one, the medicine boxes rather than a list from memory, a change of clothes, a bottle or drink, and something the child likes. Paediatric dosing is by weight rather than age, so a recent weight is worth more than an estimate. The what-to-bring page covers the rest.

Waiting with a child

Paediatric triage uses the same ladder as adult triage, with observations scored against age-specific ranges, because a heart rate that is alarming in an adult is ordinary in a toddler. Expect a re-check rather than a single assessment; expect to be asked to keep offering fluids in small amounts; and expect the department to want the child observed for a period rather than discharged the moment the fever comes down. If the child gets worse while you wait, go back to the desk and say what has changed. The triage page explains why that is the one thing that moves the queue.

Usein kysytyt kysymykset

My baby has a fever — do I go to the emergency department?

Under three months, any measured fever of 38 °C or more means calling now rather than deciding at home. From three to six months, a fever above 38.5 °C or any fever with poor feeding, drowsiness or a rash means a call. Older than that, judge the child rather than the number, and use the advice line if you are unsure.

What does the glass test mean?

Paina kirkkaan lasin sivu tiukasti ihottumaa vasten. Useimmat ihottumat haalistuvat paineessa.
