Who pays, and what the fees actually are

Six national fee systems side by side, the ceilings that limit each one, and the rule that holds in all of them: emergency care is not the thing you are means-tested on.

The rule that holds everywhere

Emergency care is provided first and settled afterwards. In Estonia it is given to everyone physically in the country regardless of insurance status; in Lithuania it is explicitly free when life is in danger or delay would cause serious harm, insured or not; in the rest, the ambulance and the emergency department do not begin with a financial assessment. This is worth knowing not because bills are pleasant but because the fear of one demonstrably delays people who should have called. It should never be the reason someone waits with chest pain.

Two ways of protecting people, and both are ceilings

The systems here divide into two designs. Denmark, and to a large extent Lithuania for the insured, make care free at the point of use and charge for what sits outside it — medicines, adult dentistry, physiotherapy. The other five charge a modest amount per contact and then cap the total: Finland and Sweden with an annual ceiling, Norway with a ceiling that triggers an automatic exemption card, Latvia with three separate caps, Estonia with a per-episode limit on the bed-day fee and a protected rate for the groups most exposed. The point of all of them is the same — to keep small charges from accumulating into a reason to avoid care — and the practical consequence is also the same: keep the receipts, because a ceiling you cannot document is a ceiling you do not get.

What is never covered anywhere

Some things fall outside every system in this family and are worth budgeting for rather than being surprised by. Repatriation from another country is not covered by any public scheme or by the European Health Insurance Card, and is the single most expensive thing that can happen to a traveller. Private rooms, non-clinical extras and better-than-standard materials are charged in most systems. Adult dentistry is largely private or partly reimbursed everywhere here. Missed appointments carry a fee in several countries. And treatment sought abroad specifically because it is unavailable at home follows the routes on the cross-border page rather than being simply reimbursed.

How each country charges the patient, and what limits it
СтранаWhat you payWhat limits it
ЭстонияSpecialist visit fee up to €20; bed-day fee up to €5Bed-day fee capped at 10 days per episode; €5 visit fee for protected groups; no visit fee if the emergency visit leads to admission
ФинляндияClient fees set by decree per serviceAn annual payment ceiling resets each calendar year; medicines run on a separate Kela ceiling
ШвецияA patient charge per visit, set by the regionA national high-cost ceiling over twelve months, and a separate one for prescription medicines
НорвегияA co-payment (egenandel) for GP, out-of-hours and outpatient careA frikort is issued automatically at the annual ceiling; under-16s pay nothing; inpatient care is free
ДанияNothing for general practice or hospital careMedicines are reimbursed on a rising ladder over twelve months; dentistry and physiotherapy are outside
ЛатвияA patient co-payment per service and per hospital day€570 a calendar year; €355 per admission at one institution; up to €31 extra for theatre procedures; under-18s exempt
ЛитваNothing for state-paid care if you hold compulsory health insuranceEmergency care is free regardless of insurance; without insurance, planned care is billed

Figures that move, and how to check them

The amounts on this page that are printed as numbers are the ones set out in a national decision and stable enough to print: Estonia’s €20 and €5 since 1 April 2025, Latvia’s €570, €355 and €31. The ones described in words rather than digits — the Finnish and Swedish ceilings, the Norwegian egenandel — are index-adjusted and change on a schedule, and a reference page that printed them would be wrong within the year. For those, the national body named on each country page is the authority, and it is a two-minute check. That is a deliberate choice: a stale number that looks authoritative is worse than a pointer that is right.

If you are not covered here

A visitor from another EU or EEA country uses the European Health Insurance Card and is charged what a local resident would be — which in Denmark means nothing at a hospital and in Estonia means the same visit fee an Estonian pays. Someone who has moved and is between systems is the genuinely exposed case: cover usually depends on registration and on contributions, both of which take time, and emergency care remains available throughout. The advice that actually helps is to sort the registration early and to hold travel insurance during the gap, because the gap is where the large bills live.

Частые вопросы

Will I be charged for an ambulance?

Emergency ambulance care is provided as emergency care in these countries, and in Estonia it is free and given regardless of insurance status. Non-emergency transport is charged differently from country to country, so the national body named on each country page is the authority.

How much is an emergency department visit in Estonia?

The specialist visit fee has been up to €20 since 1 April 2025, and it stays at €5 for the protected groups, which include children under 19, pensioners, people over 63, the unemployed and pregnant women. If the emergency visit leads to a hospital admission, no visit fee is charged.

What is the maximum I can pay in a year in Latvia?

Patient co-payments are capped at €570 in a calendar year. A single admission at one institution is capped at €355, with up to €31 more for surgical procedures carried out in theatre during that admission. Children under 18 are exempt.

Is anything free everywhere?

Emergency care is provided first everywhere in this family, and Denmark charges nothing at all for general practice and hospital treatment. Everything else varies, and every system caps what one person can pay over a year in one way or another.

Does the European Health Insurance Card mean free treatment?

No. It means you are treated on the same terms as a local resident, so you pay whatever they would pay — nothing in Denmark, the ordinary visit fee in Estonia. It does not cover repatriation, private treatment or planned care you travelled for.
