The four networks that override distance

For four groups of patients the tier system is overridden by a network, and the ambulance will drive past a nearer hospital on purpose. Stroke patients go to a hospital with a stroke unit and, where clot retrieval is indicated, to one of the few centres that perform it. Heart attack patients showing the pattern that needs the artery reopened go to a hospital with a catheterisation laboratory on call, often bypassing the emergency department entirely. Major trauma goes to a designated trauma centre with surgery, imaging and blood on immediate standby. And a very premature or very sick newborn goes to a hospital with a neonatal intensive care unit — which is also why some transfers happen before birth rather than after. In each case the extra minutes on the road buy hours of capability. The time-critical page covers the clocks these networks are built around.

Why the nearest hospital is a bad default

Choosing the nearest hospital is the right instinct for a broken wrist and the wrong one for a stroke. The mismatch happens because distance is visible and capability is not: two buildings that look identical from the car park can be an hour apart in what they can do at three in the morning. This is the strongest single argument for calling the emergency number rather than driving — the dispatcher and the crew hold the current picture of which unit is open, which scanner is working and which team is on call, and none of that is on any website. For planned care the logic reverses: there you have time to compare, and comparing is what shortens a wait, as the waiting-lists page explains.

Transfers, and what they are not

Being transferred between hospitals is routine and is usually good news about the destination rather than bad news about the origin. A county hospital that stabilises a patient and moves them to a regional centre has done its job exactly as designed. What is worth asking, as a relative, is three things: which hospital, which ward, and who to phone. Transfers are also when medication lists get lost — a patient arriving at a second hospital should have their own list with them, which is one more reason for the habit described on the what-to-bring page.

Preguntas frecuentes

Why did the ambulance drive past the nearest hospital?

Because for stroke, heart attack, major trauma and very sick newborns the destination is chosen by capability rather than distance — the stroke unit, the catheterisation laboratory, the trauma centre or the neonatal intensive care unit. The extra minutes on the road buy treatment that the nearer hospital cannot provide at all.

What is the difference between a regional and a general hospital?

A regional or university hospital carries the specialties that exist in only one or two places nationally and receives transfers from everyone else. A general or county hospital provides local acute care for a county-sized population and is organised to stabilise and transfer what it cannot finish. Both have emergency departments; what is behind them differs.

Is a bigger hospital always better?

No. For routine planned surgery a smaller hospital is often quicker, less crowded and has outcomes equal to a large one for the procedures it does regularly. Volume matters most for complex and rare procedures, which is precisely what the upper tiers are for.

Is being transferred a bad sign?

Usually not. It generally means the receiving hospital has something the first one does not, and the transfer is how the network is designed to work. Ask which hospital, which ward, and which number to call.
